Required Forms: The following 4 required forms are availible in a "physical packet" at the front office
1. A current physical (sports screening), dated after June 2009. The Physical (sports screening) is to be given by a doctor, and the information is to be completed on the form provided by the school.
2. An Emergency Treatment Card
3. A Notification of Risk Form with parent initials for each sport the athlete is to tryout for. It is recommended to initial all four sports (soccer, basketball, volleyball, and track).
4. Copy of Birth Certificate
